1. Identify youth/child that needs funding

2. Decide on funding source (with parent). If decided on KICKOFF Funding:
    a) Fill in KICKOFF application form
       *School info *Parent info *Child info
       *Sports organization info
       *Please indicate in-kind or payment at top of application.

*IMPORTANT* DO NOT SEND PARENT TO SPORT (OR REC) ORGANIZATION. TELL PARENT TO WAIT FOR VOUCHER FROM KICKOFF IN MAIL
(they will receive the voucher from KICKOFF within 2 weeks after applying)

   b) Mail (150 Exhibition St. Kentville, NS. B4N 5E3) or fax (678-4668)        application to KICKOFF.

   c) KICKOFF will then process application and notify parent of approval for        funding within 2 weeks of having received application.
